Consumer Rights & Important Information
​In accordance with Texas House Bill 4224 (89th Regular Session) and Section 181.105 of the Texas Health and Safety Code, consumers have the right to access their health care records and to file complaints regarding health care services.​
Requesting Your Health Care Records
You may request copies of your health care records directly from our office. Requests must be submitted in writing. Please contact us at admin@safespotcounseling.com for the appropriate authorization form and instructions.
